Northwest Heart Failure Clinic
For the patient with heart failure, the Northwest Heart Failure Clinic can provide the next phase for the transition of care into the outpatient care setting. The clinic provides care to assist patients with congestive heart failure (CHF) who are at greater risk for complications, re-hospitalization, poor quality of life and premature death. This clinic is designed to meet the challenge of caring for the growing population of CHF patients.

The Northwest Heart Failure Clinic provides a program focusing on improving the quality of life for patients though knowledgeable medical treatment, vital education, close follow up and support services. The staff focuses on communication and coordination of care with patients' PCP and cardiologist. When needed, the staff can provide prescription changes and medication treatments. Their goal is to give patients tools and resources to support them in maintaining a healthy lifestyle, including recognition and management of symptoms and dietary education.
